Health Center Quality Leaders achieve the best overall clinical quality measure (CQM) performance among all health centers. They are recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), or Bronze top (21-30%).
Addressing Social Risk Factors Awards recognize health centers that are screening for social risk factors impacting patient health and are increasing access to enabling services. Health centers must 1. Collect data on patient social risk factors and 2. Increase the proportion of patients receiving enabling services between consecutive UDS reporting years.
Advanced Health Information Technology for Quality Awards recognize health centers that meet all criteria to optimize HIT services. Health centers must 1. Adopt an electronic health record (EHR) system, 2. Offer telehealth services, 3. Exchange clinical information electronically with key providers health care settings, 4. Engage patients through health IT, and 5. Collect data on patient social risk factors.
Patient Centered Medical Home Recognition recognized health centers with patient centered medical home (PCMH) recognition in one or more delivery sites.
